Not fear in general — the fear that makes somebody act carefully.

唯恐 / 生怕 + an UNWANTED outcome = for fear that…, which is why the action was taken.
他走得很慢，生怕摔倒。 他反复检查，唯恐出错。
No 不 after 生怕/唯恐: 生怕迟到 (afraid of being late), never 生怕不迟到.

Plain 怕 is more neutral and can take either kind of outcome.
